To make an appointment, call our office at 614-890-0510. If you are a new patient, we ask that you complete the new patient forms online using our secure website. Our office is always accepting new patients.
We do our best to keep on time and maximize our doctor's time. If you know you will not be able to make an appointment please let us know as soon as you can. We do reserve the right to ask you to reschedule if you are more than 15 min late. A $35 fee will be charged if a 24 hour notice is not given or if you do not show up. In case of a dental emergency please call the office and you will be directed on how to proceed.

INSURANCE AND BILLING
We are pleased you have dental insurance, and our office will assist you in obtaining the maximum benefits specified in your policy. However, your insurance contract is between you, your employer and the insurance company. If you have any questions regarding your benefits, please contact your employer's Human Resource Dept. As a courtesy to you, we will file your insurance claim for you, but you will be requested to pay your estimated portion when services are rendered. Please be sure to give the front office your most up to date insurance card and inform them of any changes as soon as possible.
